- Your membership number
- Name and type of treatment provided
- Date the service was provided
- Details of the service provider
- Cost of the service
Step-by-Step guide to claiming
1. Download claim form
2. Complete all sections, print and sign the claim form.
3. Attach all receipts obtained from service providers to the completed claim form.
4. Return your claim form and receipts to us:

We have a Two Way Agency Agreement with Medicare, whereby you can leave your Manchester Unity Health Fund claim form and accounts/receipts at any Medicare office to be forwarded to us. It is not necessary to post a copy of your claim. If required we will contact you.
5. Your benefits will then be paid either by direct credit or by cheque, according to the payment method that you have selected.
Instant Claiming
Members can make EFTPOS-style automatic claims at the point of purchase using the HICAPS (Health Industry Claims and Payment Service) system. If your Extras service provider uses the HICAPS system, then simply swipe your membership card through the HICAPS terminal and Manchester Unity process all the details of your claim on the spot in seconds, meaning there is no need to fill out claim forms.
Contracted HICAPS Service Providers
General Information on Claiming
- Claims must be made within two years of the date of the service.
- If your claim has not been paid, or a receipt is not included, a benefit cheque will be made payable to the provider.
- Where you have already paid the provider, benefits will be paid to the member only, by direct credit to your financial institution or by cheque. A benefit advice will be posted to the member's address.
- If you are making a claim for Keepfit, pharmacy services or Health Dollars loyalty bonus, benefits will only be payable where the service has been fully paid by the member.
- If you are making an optical claim, your prescription details must be provided. If posting your claim, the original prescriptions will be returned to you with your benefit advice.
- If you are making a claim for in-patient hospital services, please include the top section of the Medicare statement. Please also provide the receipt if the account has been paid.
- Manchester Unity reserves the right to exclude from payment or to recover any benefit payable where, in the opinion of Manchester Unity, there is an entitlement to claim compensation or damages from another source.
